SilverOak Community Hospital in Hartford, Connecticut, is a 180-bed neighborhood institution dedicated to delivering accessible, whole-person healthcare to every resident of Greater Hartford regardless of insurance status, language, or immigration background. Established as the primary safety-net hospital for several of Hartford's most underserved neighborhoods, we are proud to be the first call for thousands of families who depend on us for everything from well-child visits to emergency care.

Our Family Medicine and Primary Care practice is the heart of the hospital, with 28 board-certified family physicians, internists, and pediatricians providing continuity care to more than 22,000 active patients. Same-day urgent appointments are available for established patients, and our walk-in urgent care clinic — open seven days a week from 8 a.m. to 10 p.m. — sees more than 40,000 episodic visits annually without requiring an appointment or insurance.

Preventive care and community health are woven into the fabric of SilverOak Hartford's mission. Our community health education programs — delivered in English, Spanish, Arabic, and Somali — cover chronic disease management, nutrition counseling, smoking cessation, maternal and infant health, and substance use recovery. Our Patient-Centered Medical Home model ensures that patients with complex chronic conditions receive coordinated care from a dedicated care team rather than fragmented episodic services.

We understand that the social determinants of health — housing instability, food insecurity, unemployment, and lack of transportation — are as consequential as clinical factors in health outcomes. SilverOak Hartford employs four licensed social workers embedded in the primary care clinics, partners with 11 community organizations for referral and wraparound services, and operates a free prescription assistance program that eliminates out-of-pocket medication costs for qualifying patients.
