SilverOak Rehabilitation Center in Portland is a 160-bed inpatient and outpatient rehabilitation hospital dedicated to helping patients recover function, rebuild independence, and return to the lives that matter most to them after disabling injury, illness, or surgery. As an accredited Commission on Accreditation of Rehabilitation Facilities (CARF) facility, SilverOak Portland meets the most rigorous standards of evidence-based rehabilitative care, and our outcomes data — submitted annually to the national Uniform Data System for Medical Rehabilitation — consistently place us among the top performers nationally on key functional independence measures.
Stroke rehabilitation is the cornerstone of our inpatient program. Our specialized stroke rehabilitation unit provides intensive therapy — a minimum of three hours of combined physical, occupational, and speech therapy daily — guided by stroke-specific recovery protocols and augmented by robotic-assisted gait training, constraint-induced movement therapy, and neuromuscular electrical stimulation. Our interdisciplinary team conducts daily morning rounds and weekly care conferences that include the patient, family members, and the full treatment team, ensuring that every decision is aligned with the patient's own rehabilitation goals and life priorities.
Traumatic brain injury (TBI) and spinal cord injury (SCI) rehabilitation are additional clinical signatures of SilverOak Portland. Our TBI program addresses the cognitive, behavioral, physical, and vocational dimensions of recovery, working with neuropsychologists, cognitive rehabilitation specialists, and vocational counselors to support return to work, school, and community participation. Our SCI program manages the full continuum of care from acute admission through home discharge, with expertise in bowel and bladder management, respiratory weaning from mechanical ventilation, pressure injury prevention, and the prescription and training of adaptive equipment including power wheelchairs and environmental control units.
Community reintegration is not an afterthought at SilverOak Portland — it is the explicit purpose of everything we do. Our therapeutic community outings program provides supervised practice of everyday activities — grocery shopping, using public transit, preparing meals, managing money — in real-world Portland environments where patients can encounter genuine challenges in a safe, supported context. Our amputee program, working in partnership with Portland's outstanding prosthetic and orthotics community, provides individualized prosthetic prescription, residual limb management, gait training, and peer visitor support from experienced amputee volunteers who know firsthand what the journey of limb loss and recovery requires.
