Biologic Therapy & Disease-Modifying Agents

Biologic therapy and disease-modifying antirheumatic drugs (DMARDs) represent the most significant advances in the treatment of inflammatory and autoimmune conditions over the past three decades. At SilverOak Health, our rheumatologists and clinical immunologists have extensive experience in prescribing, monitoring, and managing the complex safety profiles of these agents across a wide range of conditions.

Conventional synthetic DMARDs — including methotrexate, sulfasalazine, hydroxychloroquine, and leflunomide — remain the backbone of treatment for rheumatoid arthritis, psoriatic arthritis, and other inflammatory arthropathies. Combination DMARD therapy and treat-to-target strategies, guided by disease activity scores, have transformed outcomes for patients with inflammatory joint disease over the past two decades.

Biologic DMARDs targeting specific cytokines and immune pathways — including TNF inhibitors (adalimumab, etanercept, infliximab), IL-6 receptor antagonists (tocilizumab), IL-17 inhibitors (secukinumab), IL-23 inhibitors (guselkumab), and B-cell depletion (rituximab) — are prescribed for patients with moderate-to-severe disease inadequately controlled by conventional DMARDs. Our rheumatology team follows national and international guidelines to select the most appropriate biologic based on disease phenotype, comorbidities, and patient preference.

Targeted synthetic DMARDs — JAK inhibitors including tofacitinib, baricitinib, and upadacitinib — offer oral alternatives to injectable biologics and are increasingly used in rheumatoid arthritis, psoriatic arthritis, and ankylosing spondylitis. Patient education on infection risk, blood monitoring requirements, and the need to hold therapy during intercurrent illness is a critical component of prescribing these agents.

Our biologic infusion suite provides a comfortable, nurse-supervised environment for intravenous biologic infusions including abatacept, rituximab, infliximab, and belimumab. Pre-infusion assessments, real-time monitoring during infusion, and post-infusion observation protocols ensure patient safety and prompt recognition of infusion reactions. Annual biologics reviews assess response, safety, and the continued appropriateness of therapy.
