The Advanced Dialysis & Chronic Kidney Disease Centre at SilverOak Health provides comprehensive care for patients across all stages of CKD — from early detection and slowing of disease progression through preparation for renal replacement therapy and long-term dialysis management. Our consultant nephrologists, specialist nurses, renal dietitians, and social workers work collaboratively within a structured programme that optimises medical management, delays progression, and empowers patients to make informed choices about their future care.
Early CKD management focuses on targeted treatment of underlying causes — immunosuppression for glomerular diseases, optimised blood pressure control, RAAS blockade, SGLT2 inhibition, and stringent cardiovascular risk reduction — to preserve residual kidney function for as long as possible. Regular monitoring of GFR trajectory, proteinuria, anaemia, CKD-mineral bone disorder, and acidosis enables timely intervention before complications develop. Our pre-dialysis education programme introduces all modality options — haemodialysis, peritoneal dialysis, and transplantation — well in advance of end-stage disease.
In-centre haemodialysis is delivered three times weekly in our purpose-designed unit operating across morning, afternoon, and evening shifts. Haemodiafiltration — offering enhanced clearance of medium-molecular-weight uraemic toxins — is available for patients who tolerate high-volume convective therapies. Arteriovenous fistula creation and maintenance using minimally invasive surgical and endovascular techniques is managed by our dedicated vascular access team, achieving first-use fistula rates consistently above national standards.
Home dialysis programmes — including automated peritoneal dialysis, continuous ambulatory peritoneal dialysis, and home haemodialysis — are supported by comprehensive training over two to three weeks, regular home visits, and 24-hour specialist nursing telephone access. Home dialysis offers superior lifestyle flexibility, preservation of residual renal function, and equivalent or better survival compared to in-centre haemodialysis in appropriately selected patients. Our home dialysis penetration rates exceed national averages, reflecting our commitment to patient-centred modality choice.
Acute kidney injury management, including continuous renal replacement therapy for critically ill patients in our ICU, and management of resistant hypertension, fluid-refractory heart failure with ultrafiltration, and complex electrolyte disorders complete our renal medicine offering. Multidisciplinary care with cardiology, diabetes medicine, haematology, and infectious disease ensures that the systemic manifestations of advanced renal disease are managed comprehensively alongside kidney-directed interventions.
