Transplant Medicine at SilverOak Health is a highly specialised discipline encompassing the pre-transplant evaluation, organ allocation, perioperative management, and long-term follow-up of patients undergoing solid organ transplantation. Our transplant programme brings together hepatologists, nephrologists, cardiologists, transplant surgeons, immunologists, and specialist coordinators to deliver seamless, expert care across the entire transplant journey.
Kidney transplantation is the most frequently performed transplant procedure at SilverOak. We facilitate both living donor and deceased donor transplants, supported by a comprehensive donor workup programme that ensures suitability and maximises recipient outcomes. Pre-transplant education, recipient listing, organ matching, and post-transplant immunosuppression management are all conducted within our integrated transplant pathway.
Liver transplantation is undertaken for end-stage liver disease from causes including viral hepatitis, alcoholic cirrhosis, autoimmune liver disease, metabolic liver conditions, and acute liver failure. Our hepatology and transplant surgery teams collaborate closely in listing decisions, bridge therapy (including TIPSS and hepatocellular carcinoma locoregional therapy), and post-transplant surveillance for recurrent disease.
Heart transplantation and ventricular assist device implantation are offered for patients with refractory end-stage heart failure. Candidates undergo extensive physiological, psychological, and social evaluation to ensure transplantation is appropriate and that patients have the support infrastructure to manage life-long immunosuppression and follow-up.
Long-term post-transplant care focuses on immunosuppression optimisation, infection prophylaxis, rejection surveillance, and management of the metabolic consequences of immunosuppression including hypertension, diabetes, hyperlipidaemia, and malignancy. Our transplant coordinators provide continuity throughout, acting as the primary point of contact for transplant recipients and their families.
