The Gynaecological Cancer Centre at SilverOak Health provides specialist, multidisciplinary care for cancers of the cervix, uterus, ovary, vulva, and vagina within a fully integrated programme encompassing surgical oncology, medical oncology, and radiation oncology. Our gynaecological oncologists, medical oncologists, radiation oncologists, and specialist nurses collaborate at weekly gynaecological tumour board meetings to ensure personalised, evidence-based treatment planning for every patient.

Endometrial cancer — the most common gynaecological malignancy — is treated with robotic-assisted total hysterectomy, bilateral salpingo-oophorectomy, and sentinel lymph node mapping using indocyanine green fluorescence for staging. Molecular classification into POLE-mutated, MSI-high, copy-number high, and copy-number low subgroups guides adjuvant therapy selection, with immune checkpoint inhibition now incorporated into first-line advanced disease management for MSI-high tumours.

Cervical cancer management encompasses radical hysterectomy with pelvic lymphadenectomy for early-stage disease — performed robotically to minimise blood loss and enable faster recovery — and concurrent chemoradiotherapy with cisplatin for locally advanced disease. Sentinel lymph node biopsy with ultrastaging offers a minimally invasive approach to nodal assessment for early cervical cancer. Brachytherapy — intracavitary and interstitial — is integral to the definitive radiation treatment of cervical cancer, delivered with MRI-guided adaptive planning.

Ovarian cancer surgery aims for complete macroscopic cytoreduction — removal of all visible disease — which is the most powerful prognostic factor for survival. Our gynaecological oncologists perform ultra-radical surgery including bowel resection, diaphragm stripping, splenectomy, and liver surface resection when required to achieve complete cytoreduction. PARP inhibitors — olaparib, niraparib, and rucaparib — are used as maintenance therapy in BRCA-mutated and HRD-positive tumours, dramatically extending progression-free survival.

Fertility-sparing surgery — trachelectomy for cervical cancer and conservative surgical staging for selected endometrial and ovarian cancers — is offered to young women wishing to preserve reproductive capacity when oncologically safe. Vulvar cancer surgery employs sentinel lymph node biopsy to minimise groin morbidity while maintaining accurate nodal staging. Our survivorship programme addresses the sexual health, hormonal, and psychological consequences of gynaecological cancer treatment, supported by specialist nurses, psychosexual therapists, and menopause specialists.
