Maternal-Foetal Medicine Centre

The Maternal-Foetal Medicine Centre at SilverOak Health provides subspecialty obstetric care for pregnancies at increased risk due to maternal medical conditions, previous obstetric complications, or identified foetal abnormalities. Our maternal-foetal medicine specialists work within the obstetric team, collaborating closely with cardiologists, nephrologists, haematologists, endocrinologists, and neonatologists to provide truly integrated multidisciplinary management for every complex pregnancy.

Maternal conditions managed within our programme include pre-existing diabetes with vascular complications, cardiac disease in pregnancy (congenital and acquired), chronic kidney disease and renal transplant pregnancies, autoimmune conditions including systemic lupus erythematosus and antiphospholipid syndrome, haematological disorders such as sickle cell disease and inherited thrombophilias, chronic hypertension, inflammatory bowel disease, epilepsy, and psychiatric conditions requiring medication management during pregnancy.

Pre-conception counselling is offered to women with medical conditions who are planning pregnancy, enabling optimisation of disease control and medication review before conception to minimise teratogenic risk and maximise maternal and foetal safety. Detailed booking assessments for all high-risk pregnancies establish risk-stratified surveillance plans with personalised monitoring schedules and clear escalation thresholds. Pre-eclampsia prediction using first trimester combined screening (MAP, uterine artery Doppler, PLGF) with aspirin prophylaxis for high-risk cases reduces preterm pre-eclampsia by 62%.

Foetal medicine services include detailed anomaly scanning, foetal echocardiography for cardiac defect evaluation, growth and Doppler surveillance for intrauterine growth restriction, amniotic fluid assessment, and invasive testing — amniocentesis and chorionic villus sampling — with a procedural complication rate below 0.5%. In utero therapeutic interventions — laser therapy for twin-to-twin transfusion syndrome using selective fetoscopic laser coagulation, foetal thoracocentesis for pleural effusions, foetal blood transfusion for severe anaemia, and selective foetal reduction in higher-order pregnancies — are performed by our most experienced subspecialists.

Our multidisciplinary foetal anomaly MDT meets weekly, bringing together MFM specialists, paediatric surgeons, neonatologists, clinical geneticists, paediatric cardiologists, and neurosurgeons to provide comprehensive, honest counselling and individualised delivery and neonatal management planning for families facing significant prenatal diagnoses. Bereavement support and clinical psychology are integral to our care for families receiving difficult news. Our neonatal intensive care unit provides immediate expert care for premature and sick newborns born within our maternity service.
